چکیده

Pulmonary nocardiosis (PN) is an infrequent and severe infection due to Nocardia spp. It is more common in immunosuppressed individual. The aim of this study was to evaluate the clinical features, evolution and prognosis of PN in immuno-competent hosts. The study group comprised 4 consecutive immuno-competent patients with pulmonary nocardiosis acquired in a community setting, diagnosed and followed in a tertiary care hospital. Chronic obstructive pulmonary disease (COPD) and pulmonary tuberculosis were the underlying diseases. Two patients had received anti-tubercular treatment before isolation of Nocardia spp. Clinical course was chronic and diagnosis was delayed 2 weeks or more in two out of four patients. Lobar or multilobar condensation was the most frequent radiographic pattern. The disease remained localized in the lung in all the four cases, with a trend toward chronicity in one with bronchiectasis. The following conclusions were reached : (1) In our geographical setting Nocardia presents as a subacute or chronic pulmonary infection, mainly outside the hospital. (2) Pulmonary nocardiosis is difficult to diagnose, diagnosis is frequently delayed and a high level of suspicion is, thus, required in patients with underlying diseases e.g. COPD, pulmonary tuberculosis. (3) Diagnosis requires a high clinical suspicion, and can be made on the basis of a microbiological investigation of sputum or bronchoalveolar lavage. (4) The treatment of choice for this infection includes cotrimoxazole but some Nocardia spp. may show resistance to this drug.

Mycobacterium tuberculosis Dominance over Nontuberculous Mycobacteria Irrespective of Immune Status: An Indian Scenario

Introduction: In HIV-associated tuberculosis cases, identifying nontuberculous Mycobacteria (NTM) from clinical samples has become essential regarding patients’ treatment and prognosis. This study aims to determine the prevalence of different Mycobacteria species from immunocompromised and immunocompetent patients with suspected tuberculosis attending a tertiary care hospital in Kolkata, India....

Pulmonary Nocardiosis in the Immunocompetent Host: Case Series

Pulmonary nocardiosis is commonly recognized as an opportunistic infection in patients with predisposing immunosuppressive conditions. However, reports of pulmonary nocardiosis in the immunocompetent host are rare. Here, we report a case series of four patients with pulmonary nocardiosis without a predisposing condition.

Pulmonary nocardiosis caused by Nocardia cyriacigeorgica in patients with Mycobacterium aviumcomplex lung disease: two case reports

BACKGROUND Pulmonary nocardiosis frequently occurs in immunocompromised hosts and in some immunocompetent hosts with chronic lung disease; however, few reports have described pulmonary nocardiosis with nontuberculous mycobacterial lung infection. Here we report for the first time two cases of pulmonary nocardiosis caused by Nocardia cyriacigeorgica associated with Mycobacterium avium complex (M...

Cutaneous and pulmonary Nocardiosis in a patient with Pemphigus

Pemphigus is a chronic blistering mucocutaneous disease, classified in immunobullous skin diseases. Infections, particularly septicemia due to Staphylococcus aureus and Pseudomonas aeroginosa are the most common and dangerous disorders due to immunosuppressive therapy in this disease. Infection with nocardia is rare, so that only 6 cases have been reported yet. Herein, we report a 56-year-old m...

Pulmonary nocardiosis presenting as bilateral pneumonia in an immunocompetent patient – An unusual host response

Pulmonary nocardiosis (PN) is an infrequent and severe infection due to Nocardia spp., which may behave as both opportunistic and primary pathogens. The presentation of a Nocardia infection is quite variable. We report a case of pulmonary nocardiosis in an immunocompetent 24-year-old female, who was initially treated with meropenem without response. A chest radiograph revealed bilateral irregul...
